During our joint physiotherapy appointments we want to concentrate on what is important - your animal. This requires good preparation.

We save time and natural resources by giving me the data about your animal online in advance. What is the name of your animal, how old is it, what complaints does it have, what is its history?

To know all this already at our first appointment, I have created a form. You can find it under this text. By filling it out conscientiously and completely, you help me to prepare myself optimally for our conversation - and the treatment of your animal.

After you have sent the questionnaire online, I will create a digital patient file of your animal in which the master data, the treatment plan and all treatment steps are recorded. Of course you have the possibility to look at the patient file of your animal at any time. Feel free to talk to me about it.

Your data will only be used for the purpose of treating your pet and will not be passed on to third parties.
